Annual 12 month membership that allows individuals and families with no dental insurance to receive quality dental care at a reduced cost

This affordable plan includes the following services at no extra charge:

Comprehensive & six month recall exam

Annual digital xrays (4 bitewings and 2 periapicals, 1 set per year)

Two preventative teeth cleanings

Biannual fluoride treatment for children 14 and under

All other procedures will receive a 20% discount*

This plan is not dental insurance, but a cost effective way to maintain good oral health with a yearly savings of more than $100 on routine preventative care

Plan Limitations & Guidelines

Non refundable- no refunds or premiums will be issued if participant does not utilize their dental membership plan. You will not receive a membership card; your enrollment will be on file with our office.

Since this is an in-office discount plan and NOT a dental insurance plan, this program cannot be used in conjunction with another dental plan or offer. If a participant elects to use dental insurance during the year of coverage, insurance plan fees and payments will apply, and the enrollment fee will be nonrefundable.

12 month membership is due in full upon joining, effective on the day on which payment is received. Payment is due when services are rendered to receive the 20% discount. Any services not paid in full at time of service will be billed at full regular fees. Maximum discount earned will not exceed $200 annually.

It is the sole responsibility of the member to maximize benefits by arranging appointments within the 12 month membership period. If appointments are not used, the member will not be entitled to a refund.

Renewal payment is due the beginning of the same month of original joining each year. Plans and fees are subject to change yearly.

Our loyalty program is only offered to returning patients. New patients must undergo a comprehensive exam, with imaging and prophylaxis before being eligible for this program.
